We are seeking to recruit an experienced individual to work in our Intermediate Care and Rehabilitation Team, based at Purbeck Local Office in Wareham.

We have two posts available one permanent at 37.5 per week and another at 30 hour per week 1 year fixed term /secondment . Please make it clear in your supporting information which post you are applying for.

Purbeck Integrated Community Rehabilitation Team is a multidisciplinary team which includes Physiotherapists, Occupational Therapists and Nurses. We work to rehabilitate patients in their own homes and other community settings, across the whole of Purbeck, to prevent avoidable hospital admissions and facilitate earlier hospital discharge. It is essential that you have experience of patient care, preferably in community settings, as well as having good verbal, written and team working skills. You would need to be flexible in your approach and confident to work alone.  Basic IT skills and NVQ level 2/3 would be an advantage.

You will have access and support from clinical supervision, in-service training and other developmental opportunities to enable you to rehabilitate patients to their optimum abilities.

The hours of work are flexible to the needs of the service, between 8am and 8pm over 7 days a week, on a rota system.

Car owner/driver is essential, subject to the provisions of the Equality Act.
