You will work closely with a supportive multi-disciplinary team to ensure the best possible outcomes for the patients in our care.
This role involves providing domiciliary care for a wide variety of patients, offering a short term service whilst a longer term package of care is being secured. Patients will be supported in their home environment to prevent avoidable hospital admissions, facilitate earlier hospital discharges as well as in-reaching to the wards. This new role forms part of a dedicated career pathway requiring the undertaking of The Care Certificate and additional appropriate clinical training.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
