Dorset Healthcare Foundation Trust are looking for an experienced GP with specialist interest in care of the older person to work with our integrated teams in North Dorset.
A vacancy has arisen in the Shaftesbury and Gillingham locality of North Dorset for a GP Extensivist with an interest in older people to join our team. We already have a co-located Health and Social Care hub and are developing integrated ways of working that have the patient at its centre. We are actively striving to develop seamless working with our local primary care partners.
To complete this highly motivated and dedicated team of professionals we are looking for an enthusiastic GP Extensivist, to provide medical expertise for our inpatients and to work within our proactive multi-disciplinary community teams that aims to reduce avoidable hospital admissions, to facilitate community and acute hospital discharge and respond to the urgent and routine needs of patients within the community.
This is a really exciting opportunity for you to play a key role in helping to develop and shape integrated working in the Shaftesbury locality.
If you would like to work with likeminded health and social care staff and to promote innovation and excellence while putting patients at the heart of what you do – please call for more information or to arrange an informal visit.
