The Vale Primary Care Network (PCN) sets out to empower staff to be innovative and introduce pioneering changes in how we care for patients. Our focus is to ensure resources are available for proactive care of patients with long term conditions and increase capacity for acute care through integrated working of many stakeholders coming together, through shared IT and governance, enabling patient autonomy and self-care.   

Developing community hub focused care alongside primary care, we make a real difference to our community, shaping and integrating services around the population’s health needs and working closely with patients and carers to provide holistic and individualised care.

Dorset HealthCare in partnership with the Vale Primary Care Network are pleased to be able to offer to the right candidate this new and innovative role as a First Point Mental Health Worker. The role will be offered on a 2 year fixed term contract or as a 2 year secondment opportunity for existing Dorset HealthCare staff.

This is an excellent opportunity for a forward thinking and skilled mental health practitioner to develop and enhance the mental health service provided to people living in the Vale PCN. We are seeking a motivated individual who is able to develop strong working partnerships with colleagues across the Vale PCN, integrate with the existing mental health services and a range of agencies across the network.

The post holder will be an important part of the team at the Vale PCN and support the practices to achieve excellent standards of patient care. The post holder will be responsible for undertaking asset based assessments, brief interventions and care navigation for people with mental health issues living in the Vale PCN.

Employment in this post requires a Enhanced Disclosure and Barring Service (DBS) check. The DBS levies a charge of £40 and the Trust will recover this cost in instalments from your first 3 salary payments. This will only apply to candidates who are new to the Trust and not internal staff.

Applicants who subscribe to the DBS update service and are able to present a valid DBS certificate will not incur a charge.
