The Shaftesbury Integrated Community Rehab team (ICRT) is looking for a Physiotherapist to join this friendly and dynamic team. The role is primarily community based, but there may be opportunities for inpatient work within the Community Hospital

Shaftesbury ICRT is a forward thinking and innovative team. We are keen to continue to develop the Community Hub at the Westminster Memorial Hospital to provide a multi-disciplinary approach to how care services and clinical support are provided to local residents. We work very closely with Advanced Nurse Practitioners and Social Service colleagues supporting timely discharge from hospital, preventing avoidable hospital admissions and maximising independent living  As part of the role you will participate in multi-agency meetings to discuss complex patients in the community ‘Virtual Ward’ and take part in our Consultant led, Rapid Access Clinic for the elderly (RACE).

The successful candidate will have the opportunity to gain experience in palliative care, rapid response visits and undertake assessment of patients who have complex acute and rehab needs alongside management of long term conditions and provision of rehabilitation programmes.

In return you will receive support, regular supervision and annual appraisal, as well as excellent training opportunities.

Full time or part time hours available

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check. The DBS levies a charge of £40 and the Trust will recover this cost in instalments from your first 3 salary payments.  Applicants who subscribe to the DBS update service and are able to present a valid DBS certificate will not incur a charge.
