The Shaftesbury Hub based at the Westminster Memorial Hospital in Shaftesbury in North Dorset is looking for a Health and Social Care Co-ordinator to join our team. This role is an exciting and developing role across the county. It is pivotal to bring services together in order to provide seamless care for frail and vulnerable individuals in our locality and it is an exciting opportunity to be at the forefront of developments in community services
You will need to have excellent organisation skills and be a skilled communicator, as the role is integral to the development of integrated and co-ordinated services. A good level of IT experience/computer skills is required as you will be managing data on a daily basis. You would need to have previous experience in a health or social care environment and have excellent administration skills to communicate between primary care, community services and hospitals, local authority, acute trusts and voluntary services.
You will work alongside clinicians, patients, carers, statutory and voluntary agencies connected to the care of the individual patient to proactively support an effective assessment and the development of an integrated personal plan of care, to assist in avoidance of unplanned admissions and facilitate timely hospital discharge.
Employment in this post requires a Standard Disclosure and Barring Service (DBS) check. The DBS levies a charge of £26 and the Trust will recover this cost in instalments from your first 3 salary payments. Applicants who subscribe to the DBS update service and are able to present a valid DBS certificate will not incur a charge.
The hours of work will be 30 hours over 4 days which may include weekends and bank holidays. Full driving license and access to a car for work is essential as you will need to travel to meetings.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
