Are you an experienced Occupational Therapist looking for an opportunity to work with stroke patients in the community? An exciting new opportunity has arisen for a Band 6 Occupational Therapist to join Dorset HealthCare’s East Dorset Stroke Early Supported Discharge/Stroke 6-month review service on a full-time basis.
The service provides Early Supported Discharge for stroke patients in Eastern Dorset alongside the Integrated Community Rehabilitation Teams in Purbeck and East Dorset localities. The East Dorset stroke 6-month review service offers holistic support and reviews for all people with stroke in the Eastern side of Dorset, including the Purbeck, East Dorset, Poole, Bournemouth and Christchurch localities. You will be working as part of Dorset HealthCare's wider ESD and 6-month review service, which is delivered by 3 teams across Dorset who support each other. You will also have access to support from Dorset HealthCare's other services, including the Community Brain Injury Service, and an established network of neuro-specialist clinicians.
We are looking for an enthusiastic, motivated individual with excellent clinical, organisational and communication skills. The ability to work autonomously, with flexibility to meet changing service demands is a necessity.
You will be responsible for your own caseload; providing specialist assessment, rehabilitation, support and 6-month reviews for people following stroke, in their own homes and communities. You will work with the individual and all involved in their care to enable them to meet their goals. You will be working closely with Integrated Community Rehabilitation/Therapy teams, supporting other staff to deliver stroke specialist rehabilitation and 6-month reviews. You will also work with the acute referring Trusts of Poole, Bournemouth and Dorset County Hospitals, social care colleagues, voluntary organisations (particularly the Stroke Association) and others involved in an individual’s care, in order to provide high-quality input and achieve seamless transfers of care.
You must have experience of work in a neuro/stroke setting, evidence of person-centred clinical practice, and continuing professional development.
You must hold a full UK driving licence and have access to a suitably insured vehicle.
You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
