Poole Intermediate Care Service (PICS) are looking for a dynamic Team Leader to work in partnership with current Team Leader to continue to develop and progress this service.
The successful applicant will have excellent leadership and communication skills, be enthusiastic, highly motivated, flexible and have extensive experience working in the community. We would expect the right candidate to be pragmatic, solutions focused and have a positive disposition.
We are looking for an experienced practitioner, Therapist, Nurse or Social Worker.
Therapy experience or experience of working with or in Social Services would be an advantage.
PICS interfaces with a number of providers providing a high quality acute service to avoid admission and help to facilitate discharge from Acute Hospitals, through provision of a high standard of clinical assessment and development of a care and or/rehabilitation programme.
Intermediate Care Services interface with Acute hospitals, Primary Care, Local Authority and Care Providers and Community Nursing and Therapy.
It is important the right candidate can work within an integrated ethos and promote this culture within the team, and be able to drive forward the development of Integrated Community Services, take a lead with the frailty specification being rolled out in Dorset as well as having a track record in operational success.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check. The DBS levies a charge of £44 and the Trust will recover this cost in instalments from your first 3 salary payments. This will only apply to candidates who are new to the Trust and not internal staff. Applicants who subscribe to the DBS update service and are able to present a valid DBS certificate will not incur a charge.

You must have appropriate UK professional registration.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
