We currently have a number of vacancies across the Poole locality and this is an ideal opportunity for someone who is seeking to broaden their range of skills and knowledge in caring for people in their own environment as part of a multi-disciplinary team.
You will need an understanding of the current primary care , including the management of long term conditions with excellent communication skills. You will provide high quality clinical care when caring for patients in the community setting within the Poole area.
The post holder will participate in the delivery of skilled autonomous nursing care to patients within their own home, including care at the end of life, wound management, medicines management and as appropriate, assisting the patient to develop self-management skills.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check. The DBS levies a charge of £40 and the Trust will recover this cost in instalments from your first 3 salary payments. Applicants who subscribe to the DBS update service and are able to present a valid DBS certificate will not incur a charge.
