The Dorset HealthCare Medicine Team provides services across Dorset delivering to:
• Community Hospitals
• Community Hubs
• Community Services ( a wide variety of disciplines including contraception and sexual health, dental care, domiciliary cancer care services, intermediate care, district nursing to name but a few)
• Mental Health Services (ranging from acute inpatient to community mental health teams)
• Care directly into the patients home
We are looking for an experienced individual who is interested in growing their clinical repertoire in the community and are looking for the challenge of delivering medicines optimisation to patients within their own home as well as delivering inpatient care. The ambition of the Dorset HealthCare Medicine Team is to achieve a seamless transfer of patient care from secondary care through to the community setting and through to primary care. The role will involve working within a team of pharmacists and pharmacy technicians to support the delivery of integrated patient care across Dorset.
The job requires close collaboration with a number of different professions to provide holistic care that facilitates smoother transition between health providers. With the ability to provide follow up support within the patient’s home we receive referrals directly from a large variety of stakeholders and can ensure patient medicines related needs are met at every point of a their journey through the health service.
This job would suit an individual with a broad range of clinical pharmacy and pharmacy service experience that is looking for a challenge, is self-motivated, is flexible, has excellent organisational and people skills and has a track record of working un-supervised but also as a member of a multi-disciplinary team. Experience of working in an acute hospital or in a primary care setting is desirable and the individual should ideally hold, or is working towards, a postgraduate qualification in pharmacy practice or equivalent.
Due to the geographical spread of Dorset HealthCare services this role requires the individual to have their own mode of transport.
This particular post will be based out of Alderney Community Hospital in Poole; part of the East Dorset Medicines Team which covers the Bournemouth, Poole, Christchurch and Wimborne areas. The role also encompasses supporting the Pharmacy Mental Health team based at St Ann’s Hospital in Poole for one session a week.

Employment in this post requires a Enhanced Disclosure and Barring Service (DBS) check. The DBS levies a charge of £44 and the Trust will recover this cost in instalments from your first 3 salary payments.
