The Poole-based Intermediate Care Service for Dementia (ICSDE) team is looking for one senior nurse to complete our team We have one full-time vacancy for a registered nursing professional looking for fulfilling and autonomous roles where they can really use their skills, knowledge and expertise to make a positive difference to people living with dementia in East Dorset. The specialist community dementia team, which is predominantly nurse-led, offers a county-wide service alongside the ICSDW service based at Forston Clinic in Dorchester. The team supports Community Mental Health Teams in managing their most complex cases and is highly effective in preventing unnecessary admissions to the organic wards at Alderney Hospital through the provision of rapid, specialist and intensive short-term support at home. The team works closely with carers and other agencies in older adult’s health and social care services and support the discharges of people with dementia from the organic wards into community placements.

Do you have what we are looking for?

We are looking for a dynamic and highly motivated nurse, with exceptional assessment, risk management and care formulation skills who has experience of working with older adults and dementia health in a multi-disciplinary setting. A very high standard of communication skills and clinical decision-making is essential in this role. ICSDE are a ‘virtual ward’ and team leaders are expected to take charge of shifts, ‘gatekeep’ admissions to Alderney organic wards and must be able to process information in a variety of ways accurately and quickly.

Can you meet our extra requirements?

Being a car driver is absolutely vital to this role as we drive all over Dorset. The ability to work flexibly across a range of weekends, early mornings and evening shifts is also important as well as the work of the service depends on the fluidity and adaptability of the whole team. Hours of work fall between 07.30 and 15.30 as Early Shift or 12.00 and 20.00 as Late Shift.

Who do we want?

Ideally we are looking for an experienced registered nurse already able to work within a Band 6 role, RMN is preferred, but RGN or RNLD with appropriate experience and transferable skills also considered.

We have one full-time post based at Alderney Hospital within a large well-resourced and highly effective team.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check.  The DBS levies a charge of £44 and the Trust will recover this cost in instalments from your first 3 salary payments.  This will only apply to candidates who are new to the Trust and not internal staff.  Applicants who subscribe to the DBS update service and are able to present a valid DBS certificate will not incur a charge. 

You will be required to undertake a Prevention and Management of Violence and Aggression course and it is a condition of your employment that you successfully complete all aspects of this course; if this is not achieved you will not be able to commence working in the post.
