We are looking for a motivated, enthusiastic person to join our multi-disciplinary team. You will work in the ward environment alongside the Therapists, Nurses, Doctors and Social Workers to identify and facilitate discharge for patients with complex needs.
You will also help develop discharge plans and ensure timely discharge.
You will communicate daily with patient/family/carers and all health and social care professionals involved with the discharge process and ensure effective communication with regard to progress of discharge arrangement.
The successful applicant will be computer literate, communicate effectively at all levels, and will work autonomously within the scope of the role. You should be able to demonstrate excellent interpersonal and communication skills, have good time management skills and be able to prioritise and work on your own initiative as well as part of a team.
