These posts provide a unique opportunity for a diverse role with a portfolio of activities since you will work across a variety of sectors including the PCNs. The variety of services we offer to all sectors include: providing medicines management services in community hospitals, mental health units and Primary Care Networks, utilising medicines reconciliation, assessing patients medication to optimise use and supply, supporting patients with their concordance in order to achieve better health outcomes, visiting patients in their own homes in order to support them in independently and safely managing their medication.
You will possess a good knowledge of medicines optimisation which you will have already applied in a Community, Retail, Hospital, Prison or CCG setting. A good team player who is self-motivated, enthusiastic and with excellent communication skills, you should be confident and capable of working on your own as these posts involve a significant amount of lone working.
