An exciting opportunity has arisen for a highly motivated individual to join the Mid Dorset Locality team in the Health and Social Care Co-ordinator role.

The Mid Dorset Locality encompasses the town area of Dorchester and the rural areas of Milton Abbas, Cerne Abbas, Puddletown and Crossways.

The Dorset HealthCare community services, GP practices and Dorset County Council social care services are developing ways of working to deliver integrated and personalised support and intervention for patients, their families and their carers.

The role will be pivotal in the helping the wider Integrated team to co-ordinate our Multi-disciplinary team working and delivery of health and social care to the vulnerable and frail patients living in the Mid Dorset area.

The successful applicant will need to have excellent organisation and communication skills as the role is integral to the development of integrated and coordinated services. As part of the drive to improve care for service users, there may be a requirement for the post holder to assist in the response to health and social care crisis situations as appropriate and to work flexibly to cover a 7 day service.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check. The DBS levies a charge of £44 and the Trust will recover this cost in instalments from your first 3 salary payments. This will only apply to candidates who are new to the Trust and not internal staff.

Applicants who subscribe to the DBS update service and are able to present a valid DBS certificate will not incur a charge.

The postholder will have regular contact with vulnerable people and as such this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
