Do you want to make a difference to Stroke Survivors and their families/ carers once they have left hospital?

We are looking for a health care professional (Nurse or AHP), with a background in clinical stroke care to be Community Stroke Specialist Practitioner for West/ North Dorset.

Working out of a Dorchester base, you will be responsible for the day to day management of the Six Month Stroke Reviews (West/ North Dorset).You will liaise closely with the acute Trusts and be involved in the management of complex cases in the early days of returning home, performing home visits and supporting the stroke survivor and their family. You will liaise closely with the Stroke ESD services, ICRT's and the Stroke Association, as well as voluntary groups in the local areas.

Employment in this post requires a Enhanced Disclosure and Barring Service (DBS) check. The DBS levies a charge of £44 and the Trust will recover this cost in instalments from your first 3 salary payments. This will only apply to candidates who are new to the Trust and not internal staff.

Applicants who subscribe to the DBS update service and are able to present a valid DBS certificate will not incur a charge.
