As part of Dorset’s Accountable Care System journey, six million pounds are being invested in Dorset’s Integrated Community and Primary Care Services.
The aim is to develop an integrated workforce within localities, with a strong focus on partnership working. Services will be based upon the needs of people in each locality, and will focus on providing;
- Services for people with high intensity and complex needs,
- A proactive approach to care for people with medium intensity needs caused by long term health problems
As part of this investment, an exciting opportunity has arisen for enthusiastic individuals with experience in either health or social care to join our forward thinking team in West Dorset as Diabetes Support Workers.
As a Health Care Support worker (diabetes), you will be working with clinicians in our locality team to support patients with diabetes to remain as healthy and independent as possible. You will be working in people’s homes, as part of a locality based diabetes service, with support from nurses, doctors and other clinicians. Successful candidates will be willing to learn new skills and flexible to meet the needs of the service.
Core hours for this role are Monday to Friday 9am-5pm
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
