This is an ideal opportunity for someone who is seeking to broaden their range of skills and knowledge in caring for people in their own environment as part of a multi-disciplinary team.

You will need a sound understanding of the current primary care agenda, including the management of long term conditions and role as care coordinator, with excellent communication skills and effective leadership and decision making skills. You will provide high quality clinical care, supervision of staff and case management for patients in the community setting within the Blandford Area.

The post holder will participate in the delivery of skilled autonomous nursing care to patients within their own home, including care at the end of life, wound management, medicines management and as appropriate, assisting the patient to develop self-management skills.

The role also involves managing and leading a team of community nurses under the guidance and support of a Band 7 District Nurse Locality Lead.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check.  The DBS levies a charge of £44 and the Trust will recover this cost in instalments from your first 3 salary payments.  This will only apply to candidates who are new to the Trust and not internal staff.

Applicants who subscribe to the DBS update service and are able to present a valid DBS certificate will not incur a charge.
